Patient Responsibilities:
To ensure the best possible care, we ask that you:-
Provide accurate and complete information about your health history.

Inform us of any changes in your health or contact information.

Adhere to scheduled appointments or provide notice of cancellation at least 24 hours in advance.

3. HIPAA & Confidentiality
We are committed to protecting your personal health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA). This includes:

Ensuring that all health records are stored securely and only shared with authorized individuals as needed for your care.

Using encryption and secure systems for telemedicine consultations.
4. Appointments & Cancellations Scheduling Appointments:
Appointments can be made by phone and online. Appointments that are canceled before 24 hours will have rate of regular consultation.
Cancellations & No-Shows: Define the practice's policy on cancellations (are 24-hour notice) and no-show fees is applicable. If a cancelation is done prior the 24 hours the patient will receive credit if paid per consultation. No returns in copayments.
5. Billing & Payment
Billing Process: Patients will be billed either by visit or will have a member price according to the patient contract. Insurance no currently accepted.
6. Telemedicine / Virtual Consultations -
Telemedicine Availability: Integra Health MD offers virtual consultations, and the Doctor will address the consultation through our system Better Practice. A mobile phone or computer is required by the patient.

Confidentiality in Virtual Care: Assure patients that their information remains secure during virtual consultations in accordance with HIPAA.
7. Emergency Care & Referrals -
Emergency Care: Clarify the process if a patient needs emergency care and whether the practice provides emergency services or refers patients to hospitals.

Referrals: Explain how referrals to specialists or other healthcare providers will be handled.
8. Staff Conduct & Professional Standards -
Code of Conduct: Describe the professional and ethical standards expected of the healthcare providers and staff members.

Non-Discrimination Policy: Ensure that the practice follows anti-discrimination laws and offers equal care to all patients, regardless of race, gender, religion, or other protected status.
9. Medication Management & Prescriptions -
Prescription Policies: Prescriptions are managed directly to the patient or can be send directly to the pharmacy choose by the patient on its profile at the time of the registration. Medication renewals, refills, and controlled substances (in compliance with Florida's prescribing laws) will be control under supervision of the Doctor.

Patient Education: The Doctor will discuss with the patient prescription instructions, the risks of misuse, and proper disposal of medications.
10. Complaints & Grievances -
Complaint Process: Complains can be filled by email at info@integrahealthmd.com.

Resolution Process: Integra Health will address any complain directly with the personal.
Note: Integra Health MD and its Doctor has decided NOT TO CARRY MEDICAL MALPRACTICE INSURANCE.
